What changed August 31, 2025

On August 31, 2025, DHA began implementing revised regulatory controls on weight-loss medication coverage. The key changes:

  1. TRICARE For Life beneficiaries lost weight-loss GLP-1 coverage: All previously approved prior authorizations for weight-loss drugs became invalid. TFL beneficiaries must now pay 100% of the cost for weight-loss medications, even if they had an approved PA before August 31, 2025
  2. Military pharmacy fills eliminated for weight-loss drugs: Weight-loss medications can no longer be filled at military pharmacies, regardless of beneficiary category

GLP-1 coverage by drug and indication

Weight-loss GLP-1s (Wegovy, Zepbound, Saxenda)

TRICARE covers these medications for chronic weight management when prescribed by a TRICARE network provider with approved prior authorization for eligible plan types (Prime, Select, and premium-based plans).

  • Wegovy (semaglutide 2.4 mg injection): Covered for chronic weight management and cardiovascular risk reduction. Tier 2 formulary. PA required.
  • Zepbound (tirzepatide injection): Covered for chronic weight management and obstructive sleep apnea. Tier 2 formulary. PA required.
  • Saxenda (liraglutide 3 mg injection): Covered for chronic weight management. Requires PA and medical necessity form.

Diabetes GLP-1s (Ozempic, Mounjaro, Trulicity, Victoza)

TRICARE covers these medications for type 2 diabetes across all plan types, including TRICARE For Life, with prior authorization.

  • Ozempic (semaglutide injection): Covered for T2D. PA required.
  • Mounjaro (tirzepatide injection): Covered for T2D. PA required.
  • Trulicity (dulaglutide injection): Covered for T2D. Lower copayment. PA required unless documentation of another diabetes drug in TRICARE pharmacy record within the past 720 days.

Prior authorization requirements

For weight-loss GLP-1s (Wegovy, Zepbound)

TRICARE's prior authorization for weight-loss GLP-1 medications requires:

    • Adults: BMI ≥ 30 kg/m², OR BMI ≥ 27 kg/m² with at least one weight-related comorbidity (hypertension, dyslipidemia, type 2 diabetes, sleep apnea, coronary heart disease)
    • Pediatrics (Wegovy only, ages 12–17): BMI ≥ 95th percentile standardized for age and sex
  2. Step therapy requirement:

    • A 12-week trial of phentermine, benzphetamine, diethylpropion, or phendimetrazine (immediate-release or sustained-release) — OR documented adverse reaction or contraindication to these agents
    • Some TRICARE regions require trial of multiple agents before GLP-1 approval

How to navigate TRICARE GLP-1 coverage

Step 1: Confirm your TRICARE plan type

Determine whether you are enrolled in TRICARE Prime, TRICARE Select, TRICARE For Life, or a premium-based plan. This determines whether weight-loss GLP-1 coverage is available at all.

Search for your specific GLP-1 drug at TRICARE.mil to find:

Step 3: Work with a TRICARE network provider

Weight-loss GLP-1 prescriptions must be from a TRICARE network provider. Off-network prescriptions will not be covered for weight-loss indications.

TRICARE For Life: impact of the August 2025 change

The elimination of weight-loss GLP-1 coverage for TRICARE For Life beneficiaries has been the most significant recent policy change. TFL beneficiaries who were previously receiving Wegovy or Zepbound for weight management received notification that their coverage would end.

Common denial scenarios

"Weight-loss drug not covered for your plan type"

TRICARE For Life and direct-care-only beneficiaries will receive this denial. Options:

  • Switch to a covered T2D GLP-1 if you have type 2 diabetes
  • Pursue Wegovy for cardiovascular risk reduction (if you have established CVD)
  • Use manufacturer savings programs
  • Contact MOAA or your congressional representatives to support restoration efforts

"Prior authorization denied — step therapy not met"

The prescriber must document a 12-week trial of a conventional weight-loss medication (phentermine, benzphetamine, diethylpropion, or phendimetrazine) before TRICARE will approve a GLP-1 for weight loss. If the patient had a contraindication or adverse reaction, document that instead.

"Prescription from non-network provider"
