CHAMPVA — the Civilian Health and Medical Program of the Department of Veterans Affairs — provides health coverage to eligible spouses, widows/widowers, and children of veterans who are rated as permanently and totally disabled due to a service-connected condition, or who died from a service-connected condition, or who are missing in action or captured. In 2025, CHAMPVA tightened its GLP-1 receptor agonist coverage to align with FDA-approved indications only, explicitly excluding all GLP-1 medications prescribed solely for weight loss.
This guide explains the current CHAMPVA GLP-1 coverage rules, maps which medications are covered under which diagnoses, details the pharmacy benefit structure administered by OptumRx, and identifies what prescribers, benefit verification teams, and CHAMPVA beneficiaries should monitor going forward. It is independent information and not medical advice or reimbursement guidance for a specific patient or plan.

CHAMPVA program overview
CHAMPVA is not the same as TRICARE, VA health care, or Medicare. It is a cost-sharing program administered by the VA Health Administration Center in Denver, Colorado, for eligible family members of veterans. Key program characteristics relevant to GLP-1 coverage:
- Pharmacy benefit administrator: OptumRx manages the CHAMPVA pharmacy benefit, including retail pharmacy fills, home delivery, and prior authorization processing
- Meds by Mail: CHAMPVA beneficiaries can receive maintenance medications by mail at no cost through the VA's Meds by Mail program. Meds by Mail cannot be used if the beneficiary has other health insurance with prescription coverage
- Deductible: $50 per beneficiary or $100 per family per year, applied to the first medical or pharmacy claims processed
- No provider network: CHAMPVA does not have a specific provider network. Beneficiaries can see any licensed provider who accepts CHAMPVA assignment
- Cost-share: CHAMPVA typically pays 75% of the allowable amount after the deductible is met; the beneficiary is responsible for 25%
GLP-1 coverage by indication
Type 2 diabetes GLP-1s (covered)
CHAMPVA covers the following GLP-1 medications when prescribed for type 2 diabetes with a documented T2D diagnosis:
- Ozempic (semaglutide injection): Covered. Must have T2D diagnosis. Cannot be shipped to PO Box (requires refrigeration).
- Mounjaro (tirzepatide injection): Covered. Must have T2D diagnosis.
- Rybelsus (semaglutide tablets): Covered. Must have T2D diagnosis.
- Trulicity (dulaglutide injection): Covered. Must have T2D diagnosis.
- Victoza (liraglutide injection): Covered. Must have T2D diagnosis.
Coverage requires that the prescriber submits the appropriate diagnosis code with the prescription. Claims without a T2D diagnosis code will be denied.
Wegovy for cardiovascular risk reduction (MACE) — covered
CHAMPVA covers Wegovy when prescribed for the prevention of major adverse cardiovascular events (MACE) in patients with established cardiovascular disease. This aligns with the FDA's March 2024 expanded indication for Wegovy to reduce the risk of cardiovascular death, non-fatal myocardial infarction, and non-fatal stroke in adults with established cardiovascular disease and either obesity or overweight.
Documentation requirements:
- Diagnosis code for established cardiovascular disease
- Diagnosis for MACE prevention indication
- BMI documentation supporting obesity or overweight status
Wegovy for MASH — covered
CHAMPVA covers Wegovy when prescribed for noncirrhotic metabolic dysfunction-associated steatohepatitis (MASH). This reflects the FDA's August 2025 accelerated approval of Wegovy for the treatment of MASH.
Zepbound for obstructive sleep apnea — covered
CHAMPVA covers Zepbound when prescribed for a diagnosis of obstructive sleep apnea. This aligns with the FDA's December 2024 approval of Zepbound for the treatment of moderate-to-severe obstructive sleep apnea in adults with obesity.
Weight loss only — not covered
CHAMPVA explicitly does not cover any GLP-1 medication prescribed solely for weight loss. This includes:
- Wegovy for chronic weight management
- Zepbound for chronic weight management
- Saxenda for chronic weight management
- Any other GLP-1 prescribed for weight loss, pre-diabetes, or obesity without a qualifying FDA-approved indication
The VA has stated: "CHAMPVA doesn't cover prescription GLP-1 medications for weight loss."
What changed in 2025
Before January 1, 2025, some CHAMPVA beneficiaries received GLP-1 medications for indications beyond strict FDA-approved uses, including pre-diabetes and weight management. The 2025 policy change tightened coverage to align with specific FDA-approved indications only.
The change was communicated through:
- OptumRx CHAMPVA portal notice: Announced that effective January 1, 2025, CHAMPVA would only cover Ozempic and Mounjaro with a T2D diagnosis
- VA.gov CHAMPVA page update: Added a dedicated "Changes to CHAMPVA coverage for GLP-1 medications" section
- Direct beneficiary notification: Beneficiaries previously receiving GLP-1s for non-covered indications received notification that their prescriptions would no longer be filled
For beneficiaries who had been receiving Ozempic or Mounjaro for pre-diabetes or weight loss, the change meant their prescriptions would no longer be covered — even if previously approved. The pharmacy would alert the beneficiary that the prescription was not covered under their current diagnosis.
How to verify CHAMPVA GLP-1 coverage
Step 1: Confirm your CHAMPVA eligibility
Contact CHAMPVA Beneficiary Services at 800-733-8387 (TTY: 711), Monday through Friday, 8:00 a.m. to 7:30 p.m. ET.
Step 2: Verify specific drug coverage
Use the VA.gov Meds by Mail formulary search or call OptumRx Beneficiary Services at 888-546-5502 to confirm whether a specific GLP-1 medication is covered under your diagnosis.
Step 3: Obtain required documentation
For T2D coverage: Your prescriber must submit a diagnosis code for type 2 diabetes with the prescription.
For Wegovy MACE coverage: Your prescriber must document established cardiovascular disease and the MACE prevention indication.
For Wegovy MASH coverage: Your prescriber must document a MASH diagnosis.
For Zepbound OSA coverage: Your prescriber must document an obstructive sleep apnea diagnosis.
Step 4: Fill the prescription
- Retail pharmacy: Use any OptumRx network pharmacy. Present your CHAMPVA identification.

Common denial scenarios and how to address them
Denial: "GLP-1 not covered for weight loss"
This is the most common CHAMPVA GLP-1 denial. If the prescription was submitted with a weight-loss diagnosis code, CHAMPVA will deny it. The beneficiary has two options:
- Resubmit with a qualifying diagnosis: If the patient also has T2D, MASH, established CVD, or OSA, the prescriber can resubmit the prescription with the appropriate qualifying diagnosis code
- Pay out of pocket: CHAMPVA does not offer an appeal pathway for the weight-loss exclusion; it is a program-level coverage policy, not a formulary decision
Denial: "Diagnosis code does not match covered indication"
The prescriber must include the correct ICD-10 diagnosis code on the prescription. Common errors include:
- Submitting an obesity diagnosis code (E66.x) instead of a T2D code (E11.x) for Ozempic or Mounjaro
- Missing the cardiovascular disease or MASH diagnosis for Wegovy
- Missing the OSA diagnosis for Zepbound
Denial: "Prior authorization required"
While CHAMPVA's GLP-1 coverage is primarily driven by diagnosis code matching, some situations may require additional prior authorization through OptumRx. Contact OptumRx Beneficiary Services at 888-546-5502 for assistance.
What to monitor
- New FDA indications: As GLP-1 drugs receive additional FDA approvals (e.g., heart failure, Alzheimer's, addiction), CHAMPVA may expand coverage to include those indications — but this is not automatic and requires a VA policy update
- Medicare GLP-1 Bridge Program: Starting July 1, 2026, CMS will offer $50/month GLP-1 access for eligible Medicare beneficiaries through a time-limited demonstration. CHAMPVA beneficiaries who are also Medicare-eligible may benefit from this program
- Cost and access pressures: CHAMPVA coverage decisions are influenced by VA budget constraints and utilization trends. As GLP-1 costs continue to rise, coverage may tighten further
- Congressional action: Legislation could mandate broader GLP-1 coverage across federal programs, including CHAMPVA
